Participates as an active member of the interdisciplinary team, determines nursing goals of the care plan, and monitors the participant’s progress toward goals.Provides skilled nursing care and prescribed treatments to participants in their home and in clinic.Directs and supervises the provision of quality para-professional clinical care to participants in their home.Conducts customer service home visit assessments of each new participant within the first week of program enrollment and performs re-assessments and evaluations of each participant.Assist with referrals to other services when participant conditions or situations require the service of other professional disciplines or the products or services of outside companies or agencies.
