Duties
- Monitor patient health conditions and report changes to the medical team
- Collaborate with interdisciplinary teams to develop and implement patient care plans
- Maintain accurate medical records and documentation
- Conduct initial patient admissions and comprehensive reassessments in a homecare setting.
-Develop and update individualized care plans based on patient needs and clinical assessments.
-Monitor and evaluate patient progress, adjusting care plans as necessary.
-Communicate effectively with patients, families, and interdisciplinary healthcare teams.
-Ensure accurate and timely documentation of patient information and care provided.
- Driving from Clients' houses to the Office commute
Qualifications
- Valid RN license in the state of practice
- Familiarity with medical terminology and procedures
- Knowledge of hospice palliative medicine, occupational health, and family planning
- Ability to work effectively in various healthcare settings such as assisted living facilities, nursing homes, and neonatal units
Bilingual
Proven experience with patient assessments, care planning, and documentation.
Strong organizational and time-management skills.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a team.
