Job Summary
The Case Manager is accountable for collaborating with the Multidisciplinary Care Team to promote quality patient care. Facilitates efficient utilization of resources.
Identifies patient care needs throughout the continuum of care.
In coordination of care through collaborative practice – has excellent communication skills.
Knowledgeable of the Health Care Continuum and today’s challenge of change.
Staff is required to maintain a level of physical/mental health needed to perform their duties effectively.
Adheres to the Nursing Code of Ethics, St. Mary’s Healthcare Center’s Mission, Service Excellence Standards and the standards outlined in the CHI “Ethics at Work” booklet.
Education and/or Experience
Graduate from a CCNE or NLN accredited nursing program is strongly preferred. Graduate from an approved nursing program is acceptable.
Licensure/Certification/Registration
Licensed as a Registered Nurse in the State of South Dakota and/or meeting the requirements of Multi-state licensure required.
Work Schedule
Monday – Friday, 8:00am-4:30pm; 80 hours every 2 weeks
Holidays: on-call rotation
On-call: every 6 th weekend
PayThe hourly range for this position is listed below. Actual hourly rate dependent upon experience.
$29.50 - $44.25 per hour
