Description:The program offers person-centered care planning, service coordination, and support services for members receiving long-term care and/or home and community based services. The Consumer Health Advocate will provide support as a member advocate to encourage and engage members related to their healthcare.Job Duties & Responsibilities: Receiving, investigating, and resolving inquiries timely and accurately; clearly documenting all communications. Assisting members with scheduling appointments, community outreach options, necessary approvals, and coordination of care to help address GAPs in care. Working with internal and external stakeholders to obtain relevant information needed for resolution. Educating members regarding their benefits, available resources, treatment options, and navigating the healthcare system. Interact through various channels such as Chat, Phone and E-mailThis Consumer Health Advocate is also known/referred to as a PAL (Personal Assistant Liaison) for the dual enrolled Medicare/Medicaid Members. They will do a phone based health assessment with the member - initially, semiannually, and yearly.Training is Mon-Fri (remote) 8am-5pm central for 1.5-2 months.shifts open post training are: 8am-5pm mon-fri9am-6pm mon-friAdditional Skills & Qualifications:Prior Healthcare insurance knowledge a big +. Medicaid/Medicare knowledge is a big advantage. Prior call center or work from home experience is key.Outbound calls 20-50 per day.Ability to provide proof of education and employment from past.Ability to setup and troubleshoot computer equipment if/when necessary to stay online and actively making calls to member.About TEKsystems: We're partners in transformation.
