Care Management Company is seeking three qualified individuals to provide case management services to seniors in their home in the Clarksville and Nashville markets.
Currently I have one Full-time positions available in and south of Clarksville and two full time positions in the north and south Nashville areas. A part-time position may be available if the potential candidate is interested in maintaining at least 20-25 patients per month
The need around Clarksville includes Dickson, Ashland City, McEwen, Burns, Charlotte, Cumberland City and various other areas. This is a 40-50 mile radius or larger if you desire.
The company has been in business since 2015.
Prospective employees are as follows:
RN, SW, MSW, CM, MHC with one-two years of case management or home health experience.
This is a great opportunity for LPNs/LVNs with at least 3 years of experience as it would allow you to produce an income you would otherwise be unable to make at a full time job or work part-time to maintain your current pay and increase your free time!
Training and all materials needed are provided free of charge. The Case Manager will only need access to Wifi and a PC or laptop to complete charting.
Visits are one hour in length and are required one-two times per month depending on which Medicare program they fall into. Charting is required by the end of the next day when the in home visit occurred and typically takes only a few minutes per patient. The first visit is an in home assessment and this is the only visit that takes longer to chart.
If you are interested in improving the lives of your patients on more of a one on one basis then we are looking for you! As far as what is required r/t care plans, you are only required to have two care plans per patient at any one time but more if you wish.
You will be able to chart from home and make your own schedule which will allow you to work it around your other commitments and responsibilities. Case Manager can take on as many patients as you can handle to start and then when you get a feel for your job role you will be able to receive additional patients. This will allow you to transition out of your current job if that is something you are interested in. The max pre-approved caseload is 50 patients per month and anything above 50 pts is decided on a case by case basis.

Compensation is $100/Pt/calendar month if visited 2x/calendar month, otherwise visits are paid @ $50/visit and both are paid once per calendar month
Additionally, there is also a telephonic program that started in the past year and you would eventually have these types of pt's mixed in. This program compensates $20 per call with two calls completed to pts qmo equaling $40 per month per pt. These calls are usually less than 15 minutes.
