Responsible for implementing/monitoring the Person-Centered Support Plans in the day and residential programs; will be available 24 hours a day when on call.
Pay Rate:
$16.50 / hr
CASE MANAGER RELATIONSHIPS:
REPORTS TO:Case Manager Coordinator
SUPERVISES:No supervisory responsibilities
WORKS WITH: Residential Coordinators, Registered Nurse, Program Coordinator, Day Services Coordinator, Day Program Staff,Residential staff, other Case Managers, and Community Specialists.
EXTERNAL RELATIONSHIPS:Independent Support Coordinators, Residents family members/guardians, Sister Agencies for Adults with Intellectual and Developmental Disabilities, State Department of Intellectual and Developmental Disabilities, Office of Community Services, State Quality Enhancement Team/Surveyors, East Tennessee Office of Licensure, Managed Care Organizations, Department of Human Services/Adult Protective Services, physicians,and all others applicable.
CASE MANAGER RESPONSIBILITIES:
Will thoroughly learn and enforce all Sertoma Center, Inc. Personnel Policies and Procedures in an unbiased and professional manner.
Will thoroughly learn and enforce all standards as set forth by Licensure, Quality Enhancement, Medicaid Waiver, Fire Marshall, and the Center. Will serve as Officer for contract compliance in all areas.
Will be responsible for completing and monitoring all paperwork requirements of the assigned caseload in a timely manner. This includes but is not limited to the Person Centered Support Plans, Behavior Support Plans, Staff Instructions, Monthly Service Reviews, and various assessments.
Coordinate, facilitate, document, and participate in all meetings pertaining to your assigned caseload, and other meetings as may be required.
Will train all pertinent staff on individual plans, documentation and required needs of your caseload.
Will assist in the communication between Residential Coordinators, direct care staff, ISC, and center staff.
Will assist Director of Human Resources in interviewing, hiring, orienting, and training new staff.
Will be available 24 hours a day when on call as stipulated by the rotation.
Will conduct unannounced visits as directed by the Chief Operating Officer.

This will be accomplished by your efforts to control overtime expense, turnover of direct care staff, reduction in replacement costs of home inventories due to theft/misuse/mismanagement, and a dedication to safety and quality control.
You will adhere to an acceptable level of professionalism to insure the maintenance ofconfidentiality and discretionnecessary in the maintenance of client and perso nel records.
You will observe all Center routine with regard to work schedules, presence during working hours which conforms to those of the Center, designated periods of break time, lunch hour, etc.
You will strive to perform all work assignments and activities in an acceptable manner so as to require minimal supervision.
You will perform any other duties as assigned by your supervisor or the Chief Operating Officer.
RequirementsCASE MANAGER QUALIFICATIONS:
EDUCATION:Bachelors degree required or can accept 2 years college course work plus 6 years- experience with developmentally disabled (or 3 and 3 respectively).Preferred degree in Social Work, Sociology, Psychology, Special Education or related field.
WORK EXPERIENCE:One year experience working with persons with disabilities required.
LICENSE OR REGISTRATION REQUIREMENTS:Valid TN driver's license and a good driving record.
PHYSICAL REQUIREMENTS:Sight, hearing, may require some heavy lifting (i.e., toileting clients).

High School Diploma or Equivalent

Valid TN Driving Licenses
US Work Authorization
Must be at least 19 years of age
3 years of driving experience
No more than 3 moving violations on your MVR within the past 3 years
No prior felony convictions
Written and Verbal Bilingual Fluency is a plus
