Texas HCS is a provider of Home and Community Based Services (HCS) serving individuals with intellectual disabilities. The ideal candidate would have caregiver experience, and be fun, clean, mature, dependable, organized, respectful, and patient in challenging situations.
Ideal candidate in this position will be responsible for assisting the ICS Case Manager in coordinating the development and implementation of a client's Individual Plan of Care and Person Directed Plan, assisting in coordinating and monitoring the delivery of waiver and non-waiver services, assisting in arranging for transportation, and planning for a client's discharge or transfer. The Case Manager Assistant at no time will supervise staff, but only communicate staff issues to the ICS Case Manager and/or provide staff training on behalf of the ICS Case Manager .
Qualifications
â¾ A high school diploma or GED with related volunteer experience comparable to two years full-time
work in social, behavioral, or human services or related fields.
Or
â¾ A high school diploma or GED with a minimum of two years full-time work experience in social,
behavioral, human services or related work.
â¾ Be at least 21 years old
â¾ Have a valid Texas driver's license, a good driving record, and meet the requirements of insurability as
set by the company insurance agent.
â¾ Provide at least three written personal references from persons other than relatives who:
1. discuss experience in the job applying for; and
2. provide evidence for the ability to provide a safe and healthy environment for the individuals
to be served.
â¾ Provide proof of current CPR training/certification.
â¾ Have physical strength and ability to lift/support an average person if/when necessary.
â¾ No felony convictions that may bar from employment, as per regulatory standards.
â¾ No confirmations of abuse, neglect, exploitation or other actions taken which may
constitute harm to an Individual in any setting.
â¾ Sufficient knowledge of computing software including general navigation of Microsoft
Windows applications and general internet usage.
â¾ Demonstrated competency, after thorough training by Registered Nurse, to safely
perform a medication pass.
â¾ Provide writing sample sufficient to demonstrate ability to thoroughly complete all
necessary documentation.
