Work Hours: Full Time / Nights You Will Be Responsible For: · Assesses, develops, implements and evaluates a comprehensive plan of care through a multidisciplinary care team process in conjunction with the patient/family in internal and external settings. · Comprehensively assess patients' goals as well as their biophysical, psychosocial, environmental and discharge planning needs and financial status. · Procures services and advocates on behalf of the patient/family for scarce resources. · Contacts other key members of the team as needed. · Works collaboratively with staff members involved in care of case managed population. · Seeks peer consultation regarding cases that present problems/significant variances. · Acts as a liaison to post-hospital care providers and community health resources. · Manages each patient transition through the system and transfers accountability to the appropriate person/agency upon discharge. · Demonstrates ability and appropriate technical skills when administering treatments and procedures in accordance with physician's orders and nursing policy and procedure with no more than three documented occurrences per year. · Observes, documents and reports patient's response to medications as evidenced in nurse's notes with no more than four omissions per quarter. · Intervention with patients exhibiting agitated/aggressive behavior utilizes least restrictive measures available to produce desired results.QualificationsWhat You Will Need: · Currently licensed to practice as a Registered Nurse in the state of Texasor in a qualifying compact State. · NCPI Certification preferred. · Skill in assessing, planning, implementing and evaluating patient care as acquired through clinical nursing experience in an acute psychiatric setting in past year.
