Established home health agency with PHC program is looking for a Program Manager in our De-Soto Office or Dallas office. The home health agency has several locations. we had been in business for over 30 years. We provide Home Health to Medicare & Medicaid Clients, VA clients, PHC & private pay clients including RN, LVN, PT,OT, SP, MSW, HHA, Attendant, Home Maker.
JOB RESPONSIBLITIES:
1. Contact case managers for referrals PHC clients, including THHSC, Molina, Superior, Aetna, United, physicians and discharge planners by telephone, correspondence, & directly visiting there office.
2. Conduct educational training at elderly & disability facilities & group home.
3. Establish a working relationship with Visiting Physicians
4. Visit current and potential clients at their residence
5. Do client admissions as necessary
6. Do client supervisory visits per agency policy
7. Find, hire & train care givers, attendants, HHA, respite personal.
Basic Requirements:
Must have an Associate Degree in health care or business related program from a college.
Must have at least two years hands on administrative experience in PHC/PAS/ Non Medical (Private Pay) program experience working in the office and field.
Must be self starter and leader able to work with minimal supervision
Must be very fluent in verbal & Written Communication skills.
Must have excellent skills in Word Processing, Excel , and prefer use of VESTA software.
