Caring Matters is seeking a full-time Health Unit Clerk for the Nursing Department. We are looking for someone who is self-motivated, hardworking and possesses a positive work attitude. This position requires a person who is highly organized and responsible for many of the administrative duties in the Nursing Department.
1. Schedule, confirm and carry out all communication regarding medical appointments for RSS individuals served.
2. Receive monthly calendars from Host Homes for medical appointments scheduled, and enter them in the master calendar. Maintain all monthly calendars submitted for a one-year period.
3. Complete, print and provide PCFs (Professional Communication Forms) for 100% of individuals served. Clerk will always ensure accuracy of each PCF to include date, time, address of medical professional and current diagnosis using the information in the individual’s file.
4. Ensure all PCFs have all attached documents needed for the consultation as requested for review by professional (i.e. medication listing, lab reports, signed consents, etc.)
5. Coordinate and schedule lab work ordered for all RSS individuals in a timely manner. Ensure all results are received for review.
6. Coordinate and schedule all studies ordered for all RSS individuals in a timely manner. Ensure all results from studies are received for review and client file.
7. Order medications for all RSS Individuals as scheduled.
8. Create a Weekly Medical Appointment Schedule for sharing with programmatic and transportation staff.
9. Conduct weekly meeting where Medical Appointments are reviewed with programmatic and transportation staff, PCFs are disbursed and any questions may be answered.
10. Monitor for return of PCFs following medical appointments on calendar, to include any new orders for medication changes, or new medications, orders for studies if any ordered, etc. Carry out all follow-up scheduling and communication as required.
11. Assist in completing New Patient documentation for any individual served when needed; to include reporting to individual’s LAR and obtaining signatures when needed.
12. Request medical records by contacting doctor’s offices for all individuals served as needed or requested by any medical or programmatic discipline. Will secure consent to release information from nurses or case coordinators if needed.
13. Receive all Individual Profiles/Special Needs via email as updated and from each assigned Case Manager, print out for nursing dept. and ensure that each individual medical file, contains the most current Profile/Special Needs at all times. This includes updated photograph.
14. File all medical documents (PCFs, Medication orders, lab reports, etc.) as requested by LVNs weekly, each Friday by 3 p.m.
15. Assist in maintaining the integrity and organization of individual medical charts assigned.
16. Maintain confidentiality according to HIPAA regulations at all times, to include disposing of sensitive documents appropriately.
17. Providing support to the Nursing Department.
18. Other duties as assigned by RN or administration.
