415 Youngs Mill Ln, Newport News VA 23602: day care nurse LPN/ RN (part time, paid hourly), home care visit RN (part time, paid per visit).
Responsibilities
1. Day Care nurse (RN/LPN)
Assessment of day care attending participants for the admission and monthly to meet the requirement of Cardinal Care (VA Medicaid program). Need to develop and maintain effective care of clients and implement timely adjustments to service plans. Support the day care management team to ensure safety and wellbeing of participants during the day care program hours.
2. Home Care Visiting Nurse (Medicaid, RN)
Assessment of home care clients for the new admission and follow up visits at home according to Cardinal Care (VA Medicaid program). Update changes in Plan of Care according to the client’s condition, when indicated, to the case manager, and report on the quality of care provided by Personal Care Aide.
Qualifications
Minimum one to two year of experience as a RN, home health experience and/or Medicaid visiting experience preferred.
Culturally competent, open-minded nurse, hardworking nurse
Korean speaking nurse is a plus for the day care services but interpreters are on site.
