The Service Facilitator’s position is a Part-Time Independent Contractor (1099) Position. This position provides ongoing supervision of Individuals with disabilities receiving services through their insurance or a Medicaid Waiver from (DMAS) the Department of Medical Assistance Services in Virginia by conducting monthly home visits. The Plan/position requirements prefer that the Service Facilitator/Independent Contractor, possess a minimum of an undergraduate degree in a human services field or be a RN currently licensed to practice in Virginia. It is preferred that the Service Facilitator have at least two years of satisfactory experience in the human services field working with persons with disabilities or the elderly. The Facilitator must possess a combination of work experience and relevant education that indicates possession of the above suggested knowledge and skills and live in the area/counties to be served. The SF will be responsible for conducting monthly and quarterly in-person routine visits to those receiving services and care and providing written documentation via required reports on those services as required by Medicaid and their respective insurance companies. Areas to be covered are: Va. Beach, Norfolk, Chesapeake, Gloucester, Portsmouth, Suffolk an areas in between within 1 hour radius. Note pay rate is PER VISIT not PER HOUR.
