We are hiring for a Community Health Worker! This full-time, benefited position helps connect patients to high quality, affordable health care services by providing insurance enrollment, health care navigation support, and community outreach to medically under-served populations in a manner that empowers patients to understand and engage in their health care. The Community Health Worker identifies patients not actively engaged in care at UCNW and provides basic health information from a standard curriculum, connecting patients with health, social and other needed services. This position spends most of their time outside of the clinic finding and interacting with patients to help them overcome barriers which prevent them from accessing primary care. This includes working with homeless and other hard to reach populations. The position maintains knowledge of all insurance programs available in UCNWs service area and holds In-Person Assister state certification.
The ideal candidate will have:
A driver's license and reliable transportation to all service sites
Knowledge of health insurance options including Medicare, Medicaid, private insurance, and managed care programs
Knowledge of general medical practice models and processes such as office procedures, medical records, etc.
Excellent general office skills, including basic knowledge of computer and office equipment (fax, photo copier, phone system)
Knowledge of diverse populations in community
Ability to develop relationships with clients and community partners
