Growing together.Positions in this function is responsible for ensuring the continuity of care in the outpatient setting utilizing the appropriate resources and patient quality outcomes. Facilitates continuum of patients' care, with a command-center “virtual hospital” hybrid team, utilizing advanced nursing knowledge, experience, and skills.Positions in this function include RN roles (with current unrestricted licensure in applicable state) responsible for providing clinical expertise in any of the following areas:Clinical Management activities across the continuum of care (assessing, planning, implementing, coordinating, monitoring and evaluating). This includes case management, coordination of care, and medical management. Function may also be responsible for providing health education, coaching and treatment decision support for members.If you are located in Washington, you will have the flexibility to Work from home and the office in this hybrid role as you take on some tough challenges.Primary Responsibilities:
Navigating patients and families across the continuum of hospital at home care
Providing protocol driven assessment, care coordination and interventions as appropriate informed by, an external vendor
Facilitating admissions to hospital at home care

Undergraduate degree or equivalent experience
Graduation from an accredited school of nursing
Current, unrestricted nursing license (i.e. RN, LPN, LVN) in the state of Washington
3+ years of experience working inpatient within the last five years
Medical/Surgical, Medicine with IV, Home Based Care, and/or ER experience
Preferred Qualifications:
Bachelor of Science degree in Nursing
Clinical trial experience
Specialty fields such as acute care, oncology, or geriatrics
Washington Residents Only: The hourly range for Washington residents is $33.75 to $66.25 per hour.
