Patient Care Coordinator

Description We are looking for a highly organized and compassionate Patient Care Coordinator. The individual will be responsible for handling medical records, insurance verifications, patient registration, and more.

Manage and maintain patient information regarding admissions, departures, and record requests through various methods including fax, scan, and email from other healthcare facilities

Attend to all phone inquiries, gather and verify referral information, and confirm insurance details. This pertains to the admissions process and does not involve financial transactions

Review medical notes from healthcare staff, ensuring their completion and documentation in patient records

Maintain medical charts by preparing them efficiently for referrals, communication with pharmacies, and documentation during the closure of patient accounts

Methodically review patient charts for the daily census report and auditing. Verify that all records are current, complete with accurate notations and requisite signatures. Initiate follow-ups with medical staff in case of incomplete records

Requirements

Minimum of 2 years of experience in a healthcare setting, preferably in a similar role

Comprehensive understanding of medical terminology

Proficiency in electronic health records systems

Knowledge of healthcare insurance policies and insurance verification processes

Excellent communication and interpersonal skills

Strong organizational skills, with the ability to manage numerous medical records efficiently

Ability to work independently and as a part of a team

Knowledge of privacy laws and regulations such as HIPAA.
