On-Call Case ManagerOn-Call Care Coordinator
Ten-Step Hourly Range:$25.90 - $33.79, DOQ
Are you interested in working on an on-call basis and making a difference in your community? The Northwest Regional Council (NWRC) is seeking two individuals to be available On-Call to address caseloads left open due to staff vacancies or other absences.
We are an association of counties that provides services and funds community-based programs to help older adults, people with disabilities, and people living with behavioral health disorders live in their own homes and communities for as long as possible, postponing or eliminating the need for institutional care.
The Case Manager
Assesses client needs
Authorizes in-home services
Develops and implements service plans
Authorizes payment to providers as required by State standards
The Care Coordinator
Provides intensive care coordination
Comprehensive case management
Health education
Helps clients establish health action plans.
On-Call status indicates these are non-exempt, on-call, hourly positions with fluctuating hours and are not eligible for our standard benefits package. Access to our employee Assistance Program (EAP) and accrual of sick leave on a pro-rated basis are available.
