Bi-Lingual Diversion Case Manager and Service Navigator

Education and Experience Requirements:

Degree: ATA in Human Services or related field degree

3+ years case management experience

Must be bi-lingual with ability to read, write and speak both English and Spanish.

Must have the ability to work well with others on a team and independently.

Must have a strong passion for our community, serving homeless families with children, and bridging cultural gaps in services.

Must have strong organization and time management skills

Bilingual Diversion Case Manager: Diversion Service is not just a program it is a way of serving. Our goal is to reduce the trauma that families experience when faced with housing insecurities. Diversion Services allows us to serve those who are at imminent risk of losing their housing or who are already homeless. While your main priority as a Diversion Case Manager will be to help families secure stable, long-term, permanent housing, you will also help them build problem-solving skills, bridge educational gaps, decrease stress during times of crisis, and decrease the likeliness of multi-generational homelessness.

This position will require the ability to understand how to work with clients that are both situationally and chronically homeless. This role requires a highly professional, non-judgmental case manager that knows how to keep boundaries but is also willing to be innovative to find resources to serve. Some clients may be actively using or dealing with other underlying issues that are preventing them from being able to provide the home that they want for their children. It will be your responsibility/job to ensure they gain access to the service that will help during their time of crisis so that we can fulfill our mission to support them in providing loving homes for their children.

We have a growing population of Spanish-speaking clients that need the support of more staff that understand not just their language but their culture.

Service Navigator: A Service Navigator takes the time to hear the whole story to ensure that they are processed into the right programming at Family Promise. A Service Navigator specializes in diverting families from entering Shelter when possible to reduce the trauma on the family and their children, but also knows when to direct families to shelter if that becomes the best option. They need to know how to work with a family in immediate crisis, start them on goals and referral services while they wait for an opening in a shelter. This position is crucial to our ability to be able to serve every family that calls our Day Center with the same amount of care that we would give any guest that enters our Housing Programs.

PRINCIPLE DUTIES

Answer and respond to incoming calls using our triage system for intakes.

Maintain contact with families on the waitlist and refer to other resources quickly.

Provide Crisis Intervention and clear goal setting to address families’ basic needs, financial goals, occupational goals, overall family well-being, with the overall goal of helping the family obtain and keep long-term sustainable housing.

Provide weekly and bi-weekly meetings with each client on your caseloads (based on progress).

Have a working knowledge of Family Promise National and local resources, providing linkage to available services, and advocating for additional community resources.

Assess the family’s strengths, boundaries, needs, and abilities while creating a service plan directed towards permanent housing and the overall well-being of the family structure.

Advocate for all families with children that call into our center.

Maintain clear documentation of conversations, correspondence, and funding for clients.

Update communication logs, supervisor, or ED of progress monthly.

Skills with database entry are required; you will be entering all Service Navigation and case management information into HMIS, FP Force, and other tracking systems when necessary.

You must know when and how to report to your supervisors, ED, CPS, or the Police when you feel there is a danger to a child or other individual in a house.

Follows and completes other jobs as assigned by the Executive Director. Every employee at Family Promise has what we call a 10% other duties job. We do our best to share the load and do everything with a team approach.

Compensation

This position starts at $22-23. an hour 10-15 hours with the possibility of quickly becoming full-time as funds are available. Insurance offered after 90 days of working 34 hours or more.

Pay: $22.00 - $23.00 per hour
