Position Summary
The Home Care Coordinator works under the general supervision of the Clinical Director and is responsible for organizing the plan of care for participants while providing education and eliminating any potential barriers to treatment. The Home Care Coordinator monitors the quality of care and services being provided to program participants in the home. Ensures continuity of home care service delivery at the time of enrollment and throughout participant\'s members as well as compliance with all governing regulations and agency policies and procedures.
Responsibilities
You will provide skilled occupational therapy in accordance with the plan of care
You will assist the IDT in the evaluation and assessment of the patient\'s functional level by applying appropriate diagnostic and prognostic procedures and tests
You will recommend orthotics as indicated by condition and needs
You will treat the participant through the use of therapeutic activities designed to restore function and self-care activities for the purpose of improving function
You will observe, record, and report the participant\'s reaction to treatment and any changes in the participant\'s condition to the physician, PACE Clinical Director or designed, or IDT as appropriate
Requirements
Registered by the American Occupational Therapy Association or successfully completed the educational requirements of occupational therapy
State of Washington Occupational Therapist license
Health Care Provider level CPR certification
Minimum one (1) year of occupational therapy experience
Minimum one (1) year of experience working with frail or elderly population preferred
Experience in Medicare Certified Home Health preferred

Pay and Benefit Expectations
We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $94,515.00 - $139,402.00 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. What you will do:
Responsibilities listed include, but are not limited to, the following:
Conducts full-cycle recruitment to include, job advertising, sourcing, interviewing, and processing pre-employment background checks, and onboarding
Represents the local branch in job fairs, on-site hiring events, and build solid partnerships within the local community
Implements and adheres to company policies and complies with state and federal laws and regulations to ensure HR compliance
Enhances the candidates experience throughout the process in a timely manner and adheres to service level agreements (SLAs)
Maximizes best practices in recruitment strategies and diversifies candidate outreach efforts on social media and job platforms (LinkedIn, Facebook, Handshake, Indeed, MyCNAjobs, etc.)
Collaborates with regional talent acquisition and human resource teams with on-going staffing needs and maintains up-to-date employment records
Utilizes the applicant tracking software (iCIMS) and completes data entry within payroll and schedule tracking systems
Coordinates required trainings, confirms team member certification requirements, and conducts periodic audits of employee files

Qualifications
What you will need:
High school diploma or GED is required
Some college courses completed or a two- or four-year college degree is preferred
Minimum of one year of office administration, recruiting, and/or human resources experience
Knowledge and experience with Microsoft Office software
Experience with an applicant tracking system or data entry software is a plus
Effective verbal and written communication
Capable of working responsibly with confidential information
Accountable, reliable, and ability to work independently with good judgement
Successful completion of pre-employment background check
