Provides case management and other various clinical services as needed.

Assists in answering the Community Housing Network/Coordinated Entry Homeless Phone line to either refer to resources, conduct an HMIS assessment over the phone or scheduled an appointment to complete an HMIS assessment face to face.

Assists in completing HMIS and other housing related assessments and/or paperwork.

Assists in supporting the Community Housing Network/Coordinated Entry program by attending all assigned meetings and/or trainings.

Assists in supporting other Catholic Charities Housing Support Services programs by serving as back up if needed for any urgent or emergent contacts.

Participates in other Housing Support Services Programs meetings, trainings, and webinars, or conference calls.

Assists in developing and providing a range of integrated and varied life skills training workshops designed to assist individuals in the acquisition of skills, retention of current functioning, or improvement in the current level of functioning, appropriate socialization, and adaptive coping skills. Assists in facilitating at least one workshop offered daily Monday-Friday and or incorporating these skills for short term case management purposes for any Catholic Charities Housing Support Services programs.

Assists in supporting other Catholic Charities Housing Programs by setting up appointments, answering office phone, taking messages, assisting in keeping the work areas clean and problem solving areas or needs for improvement within the program.

Assists with Supervisor in making sure there is adequate coverage available for the Community Housing Network Phone line and walk in program

Works with Catholic Charities Housing Support Services team members and the supervisor to secure coverage for all hours of the Monday through Friday 8 a.m. to 5 p.m. workweek.

Advocates and fundraises for Housing Support Network to gain donations that can be used directly to support the program and people who participate, picks up donations as needed.

Coordinates and communicates any changes in Community Housing Network with the reception and support staff as they occur for continuity.

Provides quality housing related services and refers clients to the appropriate mental health services for consumers with a mental health disorder.

Completes progress notes and other required documentation in the electronic consumer files within the required timeframe.

Provides services to or on behalf of the person served

Services may be provided in the community or in one of the program locations; they may include crisis intervention, assisting with applying for benefits, activities of daily living, psychosocial support, service coordination, linkage, transportation, accessing community resources and/or housing/housing retention.

Establishes and maintains working relationships with community resources, coordinates consumer treatment with other providers, and initiates referrals to medical providers and other community resources when appropriate.

Performs other duties as assigned

Education:

High School Diploma or GED

Bachelors degree in Behavioral Sciences or Social Work preferred

Associates degree in Behavioral Sciences with two years experience

Experience:

Two years experience in direct clinical treatment services preferred

At least one year experience in the treatment with seriously disturbed adults/children and their families

Special Skills:

Strong family and individual clinical case management skills.

Possess and utilize effective verbal and written communication skills

Ability to be self-motivated, work independently and as a team member

Ability to represent the agency in a professional manner within the community

Bilingual/bi-cultural (English/Spanish) preferred, but not required

Licensure, Registration, Certification:

Valid Washington State drivers license and required minimum liability insurance

Agency Affiliated Counselor Registration

Employment is conditional upon:

Being cleared by criminal background check and fingerprinting when required

Must be at least 18 years of age
