Job Title: Outreach Coordinator / Case Manager

General Purpose:

Demonstrate Christ-like behavior and attitude in all job duties and responsibilities.

Responsibilities/Duties:

Provide outreach and case management services to people experiencing homelessness with chronic mental illness and substance abuse issues. Serves as a member of the organization that welcomes, engages, and serves individuals with complex wellness needs.

Provide outreach and case management direct services to chronically mentally ill homeless persons.

Assume responsibility for assigned consumers, ensuring follow-up on referrals for necessary services and facilitating continuity of care.

Participate in intake assessments and treatment planning for consumers.

Provide an inventory of housing resources and working agreements with local housing providers for increased accessibility to housing resources.

Provide education to facilities, professionals, and other caregivers.

Ensure that ethical and professional standards of practice are maintained.

Maintain records and provide reports in accordance with policies and procedures.

Participate in Adult Services meetings and activities as required. Coordinate work with other staff

Demonstrate an ongoing commitment to increasing competence and knowledge in the mental health field.

Supervisory Duties:

Orienting and training volunteers in outreach services. Planning, addressing complaints, and resolving problems.

Minimum Qualifications:

The following are the minimum qualifications that an individual needs to successfully perform the duties and responsibilities of this position. Please note that the minimum qualifications may vary on the department size and/or geographic location.

Knowledge:

High school diploma or GED;

People skills to interact with a variety of other people, such as volunteers, program participants, management, and sometimes donors.

Ability to stand in one place for hours and at the time, handle extreme heat and cold.

One-year relevant experience in the homeless outreach services-related field.

Experience in the provision of case management services that demonstrate the ability to work with the mentally ill homeless population is preferred.

Valid driver’s license

Bilingual English/Spanish is a plus

Ability to pray for someone openly and share the gospel when asked.

Must be able to see, hear, speak, and write clearly to communicate with employees and/or other customers; manual dexterity is required for occasional reaching and lifting of small objects, and operating office equipment.
