The Eau Claire City-County Health Department has an exciting opportunity for a part-time Public Health Nurse in the Healthy Beginnings Division. The Healthy Beginnings Divisions provides a wide variety of individual and family services that positively impact the health of our community, specifically the maternal and child population. As a member of the public health nursing team, your primary duties will be to:

Provide comprehensive nursing services to women and their families eligible for the Nurse-Family Partnership Program through assessment, teaching, counseling, referral and case management.

Adhere to the nursing process and the Nurse-Family Partnership model of home visitation.

Provide nursing services in homes, clinics, schools and other community sites to protect and promote the health of the public. Coordinate and collaborate with community resource organizations to promote continuity of care.

Collaborate in development and delivery of programs and activities for individuals, families, and populations groups.

Create program reports as requested for grants, Medicaid, etc., and computerized clinical records and time reporting.

Provide public health nursing expertise and leadership in community and system initiatives to promote access to needed health services for all; promote healthy lifestyle choices and institute measures to reduce health disparities for at-risk populations in the community.

Serving in this position, you may also:

Serve on interdisciplinary department teams planning, implementing and evaluating community health promotion activities.

Participate in quality improvement projects to improve department services and processes.

Assist with data collection, grant writing and evaluation of contract and grant programs.

Provide presentations to community and academic groups about topics of interest pertaining to public health nursing.

Institute measures to prevent and control communicable disease such as administering immunizations.

Training and Experience Required:

1. Baccalaureate degree from a college or university school of nursing which is accredited by the National League for Nursing or the Commission on Collegiate Nursing Education.

2. Current Wisconsin license to practice as a registered professional nurse.

3. Public health nursing/home visitation experience preferred.

Ability to communicate with individuals to exchange basic information and situation status, to consult on technical issues, and to address concerns and issues involving nursing and public health issues.

Skills and abilities of conflict resolution and effective listening to address concerns of distressed persons and stressful situations.

Ability to effectively communicate the level of risk, in each situation, based on nursing judgment and technical expertise.

Public sensitivity for working with residents and organizations affected by controversial health-related issues.

Ability to professionally handle confidential information.
