Build a professional one-on-one working relationship with clients to aid in eliminating challenges that are hindrances to obtaining and retaining employment. This includes assessment, individualized attention and coordinationPrepares, organizes, and maintains accurate, updated information on both electronic and paper participant files reflecting the entire history of a program participant including log of supportive services issuedProvides case management with a focus on helping customers to prepare for employment that leads to self-sufficient. Maintain organized database of clients and keep detailed documentation of all contacts with clients - Ensure participants follow through with planned activities as outlined in contract with funding agencyProvide individualized assessments of work, provide referrals to appropriate community service or social service agencies when neededAccurately complete required documentation for enrollment and ongoing case managementAssist clients with labor market information, identifying education and training requirements, and selection of appropriate training provide. Gather documentation and prepare vouchers for training and supportive services Document all participant contacts and activities in case notes and ensure that post-placement contracts are made as required by specific contractCreate solutions to participant attendance problemsComplete project reports and meet report deadlines. Participate in activities including training, capacity building, and professional development - Work with leadership to identify operational challenges and to design and implement process improvementsProvide operational support and coverage as neededAssist with Intake/registration when needed Provide operations support for all compliance driven standardsMay be assigned other tasks and duties reasonably related to their job responsibilitiesOther duties as assigned
