Duties Included
- Evaluate the client’s needs and preferences for services and make referrals to other services as appropriate.
- Develop a plan of care for the client.
- At least once every 50 to 60 days, the RN supervisor will visit each client’s home and, based on the home visit, complete the following activities::
- Review and evaluate the client’s medical condition and medical needs per the written Plan of Care during the period in which care is being provided.
- Determine whether the current level of services, including frequency and duration of service, continue to be appropriate to treat the client’s medical condition.
Other duties as assigned by the Administrator.
