The utilization review/discharge nurse will work with multiple departments and stakeholders to ensure optimal patient outcomes. This position will evaluate the patient's current condition to assess the appropriate direction of care and make recommendations to the providers based on their assessment. Provides, coordinates and facilitates patient discharge planning in collaboration with other health care professionals during hospitalization, swing bed, outpatient and emergency room visits.
MINIMUM QUALIFICATION STANDARDS:
Education: Graduate from an accredited nursing program with a bachelor degree.
Licensure: Current and active Registered Nurse license in the State of Wyoming
Work Experience: 3 or more years' experience as a Discharge Planner or Utilization Review nurse.
Free from illegal use of drugs, and freedom from use and effects of use of drugs and alcohol in the workplace.
Persons who have been found guilty by a court of law of abusing, neglecting or mistreating individuals in a health care related setting are ineligible for employment in this position.
