Care Transitions Coach - This full time position is based in the Grass Valley Office, co-located at Sierra Nevada Memorial Hospital
FREED is seeking qualified candidates for the position of Care Transitions Coach. As a member of FREED’s Transitions Team, The CTI Coach works in collaboration with and the Aging & Disability Resource Connection (ADRC) and other members of FREED’s service team, to implement the Care Transitions model and Hospital to Home transitions services. This position is co-located at Sierra Nevada Memorial Hospital and at the FREED office in Grass Valley. EOE, Individuals with disabilities are strongly encouraged to apply.
