The Youth Intervention Specialist works closely with the School Based Wellness Program Director, and school administrators to ensure operation procedures are established and maintained for the Student Wellness Center. The Youth Intervention Specialist provides assistance to students and families accessing resources from The Humboldt IPA Wellness Centers.
Responsibilities:
Assist in the day to day activities as necessary to ensure wellness center operations are performing as expected
Assist in data analysis to support the wellness centers. Maintains data to support goals and objectives
Able to advocate student needs related to health issues, infrastructure, and programs
Assists in the enrollment process for all students
Develops goals in partnership with students and families to support the wellbeing of the entire household
Refer students/families to community resources as necessary
Complete SDOH Screenings and Bright Futures screenings as necessary to develop goals with reoccurring students
Provides assistance to school site administrators, teachers, and staff
Complete home visits with students and families as needed
Assist with and participate in activities to support health promotion in the classroom including empowerment groups, and other events
Participate in activities to support enrollment in wellness center services, such as new student orientation, welcome back events, Back to School Night, Open House, etc.
Initial outreach to parents/student upon enrollment, welcome, very demographics
Provision in monitoring wellness center supply inventory system to ensure necessary supplies such as laundry soap, clothing, snacks, toiletries, etc.
Document 1:1 school site sessions, or home visits in ECW or other platforms as applicable.
Data entry
Answer, refer and/or follow-up via telephone, fax, email and mail with parents questions related to the program and specific needs of the students and families
Keep up to date with current community recourses available for students and parents
Other duties as assigned
Qualifications
Qualifications:
Project coordination experience; working on projects across organizations preferred
Experience in a professional environment and structure
Present a professional image at all times in public and within a work setting
Experience working with children, adolescents and young adults
Knowledge of the community and school’s populations culture
Strong communication and writing skills
Strong presentation skills
Ability to be a self-starter, quick learner, prioritize effectively, organize workload, meet deadlines, be flexible, pay attention to detail, work independently and function effectively as part of a team
Experience with working across organizations and/or stakeholders
Proficient computer skills, including Microsoft Word, Excel, and Power Point,
Compassion, patience, flexibility, curiosity, initiative and accountability
Valid CA Driver’s License
