Hospice of Humboldt is now accepting applications for a Full-time Intake Coordinator.
The Hospice Intake Coordinator is responsible for coordinating the process by which patients are admitted to Hospice. This position works closely with the Senior Intake Coordinator, supervising Clinical Manager, Admissions clinicians and other hospice staff to ensure timely, efficient and successful admissions.
This position is full-time 40 hours/week and comes with full benefits. The Hospice Intake Coordinator is also required to be on-call for hospice admissions intake needs for up to three (3) holidays per year. The on-call shift is 0830-1700 weekdays.
Minimum Qualifications -
1. Minimum of one (1) years’ experience in a healthcare setting.
2. Familiarity with medical terminology and diagnosis
3. Excellent communication and customer service skills
4. High orientation to detail and ability to multi-task
5. Flexibility and team-oriented work style
6. Ability to serve as a positive representative of the agency and act in a kind, respectful manner
7. Valid driver’s license, proof of acceptable automobile insurance coverage and reliable transportation as specified by Hospice of
Humboldt’s policies.
8. Excellent computer skills.
9. Proficiency in Microsoft Office applications.
