We are hiring a part-time, 24-30 hours per week, Nurse Care Manager in our MSSP program. Our Nurse Care Manager evaluates and provides care management services to older adult participants, identifying both biopsychosocial-spiritual strengths as well as limitations which impede independent living. Conducts in-depth health assessments/reassessments and completes Level of Care determinations to certify/recertify participants for the program. Must have an associate or bachelor’s degree in nursing from an accredited nursing school and a minimum of one (1) year of clinical experience, preferably with older adults.
Experience with nursing care/case management and/or community-based nursing preferred. Must be a current Registered Nurse licensed by the California Board of Registered Nursing, in good standing. Current BLS/ACLS within six (6) months of hire. Valid California Driver’s License, and personal auto liability insurance required. Must have a personal vehicle in good driving/operating condition and insured for State minimum liability requirements that can be used for the applicable job functions.
