Redwoods Rural Health Center is seeking a Registered Nurse CCM/Telehealth Coordinator to join our team! The Registered Nurse works under the direct supervision of the Medical Director(s) and has a close daily working relationship with the Medical Team. Responsible for nursing patient care services, triage, population health programs such as CCM and Telehealth, as well as supporting Medical Assistant training, as needed.
