Hospice of Humboldt is accepting applications for a Per Diem Admissions Nurse.
Under physician’s orders and in cooperation with other Interdisciplinary Group members as described in the patient’s Plan of Care, the Admissions Nurse is responsible for identifying needs at the time of admission and coordinating care provided to the patients in the home, SNF, and RCFE settings.
The Per Diem position works a minimum of 2 shifts per month, flexibility to assist with Admissions needs is desired.
Minimum Qualifications are as follows:
1. Graduate of an accredited nursing program.
2. Current California Registered Nurse license.
3. Minimum of one year of experience as a professional nurse within the previous three years or equivalent experience
in a BSN program.
4. Valid driver’s license, proof of acceptable automobile insurance coverage, and reliable transportation as specified by
Hospice of Humboldt’s policies.
5. Reliable mobile phone with plan that provides good local coverage.
6. Excellent computer skills.
