Remi Vista, Inc. is a local non-profit now hiring a Mental Health Program Coordinator. Program Coordinator oversees the Child Rehabilitation Specialists. This position requires a bachelor's degree in related field and two years of experience is preferred. Background clearance, physical/TB/drug screen, and DMV record with full use of vehicle required. Some duties include:
Accept referrals from supervisor and conduct home visits of referred clients, providing assessments and intervention plans using approved tools in a timely fashion, and assure all plan identified needs are met.
Create individualized treatment plans for clients behavioral goals
Provide direct intervention services to children, youth and families in their schools, homes, and communities.
Provide interventions for children, youth and families that support the individualized Family Plan and is evidenced in audit-ready progress notes.
Provide oversight of billing and progress notes/ documentation.
Train, supervise, and schedule of direct support staff.
Work with referred families, representatives of referring agencies, and other relevant parties (in-house and external) including schools, social services, mental health, probation, public health and others to facilitate the familys need to access appropriate community-based resources
Assure completion of all required documentation with regard to the services delivered and document these services in clients records.
Maintain positive, open communication with staff, clients and community agencies.
