DUTIES:
o Makes visual observation of an area to ensure consumers’ whereabouts and safety, and monitor activities.
o Attends to the personal care needs of consumers: bathing, hair care, nail care, shaving, diapering, toileting, dressing, oral hygiene, etc.
o May manage disruptive behaviors.
o Keeps required records on consumers’ activities and behavior, physical condition, and reactions to medications and treatment, based on observation.
o Interacts with consumers at mealtimes, assist in feeding consumers when needed, and maintains records on food and liquid intake if necessary.
o Assist consumers into and out of wheelchairs, beds, and other equipment.
o Transports and/or accompanies consumers to appointments.
o Discusses consumer progress and care plans with ID team members and consumers’ family members.
o Performs light housekeeping task such as sweeping, mopping, disinfecting wheelchairs, making beds, taking the trash out, etc.
o Changed bed linens, operates washing machine and clothes dryer; sorts, folds and puts away clothing.
o Sets up and administers medications.
o Scheduled and participates with consumers in recreational and therapeutic activities.
o Monitors consumers who are likely to be physically abusive to others or to self.
o Attends in-service training programs.
