Job Overview

MUST HAVE HOME HEALTH EXPERIENCE!

We are actively seeking a Registered Nurse with home health experience to join our team and provide exceptional care to patients newly discharged from the hospital. This role focuses on delivering complex care and education to help patients transition successfully from the hospital to their homes. The primary goal is to teach and empower patients and their families to manage their care independently.

Key Responsibilities:

Patient Education: Teach patients and their families how to manage medications, wounds, and medical devices, with a focus on fostering independence and confidence.

Wound Care: Perform advanced wound care, including managing surgical wounds, pressure ulcers, and complex chronic wounds.

IV Therapy & Infusions: Administer IV antibiotics, hydration, and other infusions; manage and educate on various IV devices, including PICC lines, midlines, and peripheral IVs.

Care for Central Lines and Ports: Provide expert care and training for central venous access devices (CVADs).

Ostomy Care: Educate and assist patients with ostomy care and maintenance.

Trach Tube and J-Tube Care: Provide skilled care and education for tracheostomy tubes and jejunostomy tubes.

Pain Management: Evaluate and manage patient pain, ensuring adjustments to care plans as needed.

Catheter and Drain Management: Manage urinary catheters and surgical drains, ensuring proper function and comfort.

Monitoring for Complications: Assess for infections, signs of poor circulation, adverse reactions, and other complications, intervening as necessary.

Collaboration with Healthcare Team: Communicate regularly with physicians, LVNs, and other care team members to ensure optimal patient outcomes.

Additional Notes for RN:

No OASIS Documentation Required: Focus entirely on clinical care and following physician orders without the burden of OASIS assessments.

Supervisory Role: Provide guidance and oversight to LVNs to ensure proper patient care.

Requirements:

Minimum of 1 year of home health nursing experience (required).

Current RN license in CA.

Expertise in managing complex medical needs in a home setting.

Strong critical thinking, clinical, and assessment skills.

Reliable transportation and valid driver’s license

CPR/BLS certification.

Rates vary based on visit type: higher rates are offered for Start of Care (SOC) visits, and standard rates apply to follow-up visits.
