Our mission is to facilitate residents' recovery through compassionate care and professional medical support.
Responsibilities:
Monitor Vital Signs and Assessments: Regularly check and record residents' vital signs—including blood pressure, heart rate, temperature, and respiratory rate—to evaluate their health status and promptly report any significant changes to the healthcare team.
Wound Care: Perform wound assessments, cleaning, and dressing changes, ensuring proper infection control practices and promoting optimal healing.
Medication Management: Administer prescribed medications accurately and timely, monitor residents for potential side effects, and maintain meticulous medication records.
Patient Support: Assist residents with daily activities as needed, provide education on health management, and offer emotional support to residents and their families.
Documentation: Maintain accurate and up-to-date patient records, documenting all care provided, observations, and any changes in residents' conditions.
Qualifications:
Active LVN or RN license in California.
Experience in post-acute care, rehabilitation, wound care, or home health is preferred.
Strong communication and organizational skills.
Compassionate, patient-centered approach to care.
