REGISTERED NURSE (RN) FIELD NURSE
JOB SUMMARY:
The Registered Field Nurse is responsible for providing direct patient care under the supervision of a Case Manager and/or Director of Patient Care Services. He/she must be able to function independently in performing the nursing process - from initial evaluation and assessment through planning, implementation and evaluation of the Plan of Care. Proficiency in OASIS-C is required as is the submission of timely documentation. Familiarity with web-based clinical documentation tool Synergy is a plus.
JOB DUTIES:
- Conducts in-home patient evaluations.
- Provides direct patient care.
- Implements plan of care initiated by the clinician.
- Provides accurate and timely documentation consistent with the plan of care.
- Provides patient and family/caregiver education.
- Participates in coordination of Home Health services appropriately reporting the
identified needs for other disciplines (i.e. HHA, PT, SW, etc.)

REQUIREMENTS & QUALIFICATIONS:
- Graduate of an accredited nursing program.
- Minimum 1 year experience in home health, hospice or related field.
- IV Proficient (including chemo) a PLUS.
- Wound Care Certified/Proficient a PLUS.
- Psych certified is a PLUS
- Current CPR.
- Professional Liability Insurance.
- Currently licensed as a registered nurse in the State of California.
- Demonstrates good verbal and written communication and organization skills.
- Spanish Speaking, preferred but not required.
- Valid Driver License.
- Active Car Insurance.
- Physical/TB Test must be within 6 months.
PHYSICAL DEMANDS:
This position requires good enough overall physical and mental health to handle the physical and emotional demands of caring for patients.

LICENSED VOCATIONAL NURSE (LVN) Looking for an LVN that can do home health
DEFINITION : The Licensed Vocational Nurse provides direct nursing care to clients as prescribed by the Physician, and under the direction and supervision of the Registered Nurse. Nursing activities are limited to those in compliance with the State LVN Practice, any applicable licensure/certification requirement(s) and agency policies and procedures.
QUALIFICATIONS :
- Graduate of an approved School of Vocational Nursing.
- Must be currently licensed (in good standing) in State(s) of nursing practice.
- One (1) year experience as a professional nurse.
- Good emotional health.
- Must meet educational/experience requirements outlines in the laws of licensure states.
- Must be able to verbally communicate in the language of the client and document all client related activity practice.

JOB DESCRIPTION :
Provides direct skilled nursing care.
Administers medication as prescribed by the Physician as permitted by law.
Assists in the evaluation of effectiveness of treatments and monitors client/patient's reactions
Contributes to the coordination of services in conjunction with other members of the health care team.
Maintains accurate records.
Reports significant findings or changes in the client/patient's condition to the Registered Nurse and client/patient's Physician.
Records clinical progress notes in client/patient's clinical record no less often than weekly.
