Hospice Visiting RN:
- Comprehensive assessment - SOC and Recertification of patients and family to determine hospice needs. Completes
physical assessment and history of current and previous illnesses.
- Completes the assessment in "HOSPICEMD" software in a timely manner.
- Develops a care plan that establishes goals, based on nursing diagnosis and incorporates
palliative nursing actions. Includes the patient and the family in the planning process
- Counsels the patient and family in meeting nursing and related needs
- Provides health care instructions to the patient as appropriate per assessment and plan.
- Can be available for RN Supervisory visits or PRN visits, when needed.
Hospice Visiting LVN:
- Follow up visits
- Completes visit notes in "HOSPICEMD" in a timely manner
- Can be available for continuous care or PRN visits, when needed.
