Overview
Join our team as a Licensed Vocational Nurse (LVN) where you will have the opportunity to coordinate care, obtaining and processing orders, documentation from our head quarter located in Woodland Hills. Duties
-Obtain MD orders and coordinate with pharmacy for delivery.
-Review documentation.
-Join weekly IDT meetings.
- Administer medications and treatments as prescribed by healthcare providers if needed.
- Monitor and assess patient conditions, reporting any changes to the medical team
- Collaborate with physicians and other healthcare professionals to develop and implement patient care plans
- Assist in patient education regarding health management and preventive care
- Maintain accurate and detailed patient records in compliance with regulatory standards
- Utilize eDSL or similar electronic health record systems efficiently
- Utilize Micro-Soft word and excel spreadsheet.
Qualifications
- Must hold a valid LVN license in CA
- Experience in hospital or primary care settings is preferred
- Familiarity with acute pain management techniques is advantageous
- Knowledge of health coaching principles is a plus
- Ability to work effectively in fast-paced environments while adhering to industry standards
- Experience in memory care or internal medicine is beneficial
- Willingness to travel for assignments if required

Pay: $28-33 an hour
