Under the supervision of program director,
A nurse will be
1) monitoring the health condition of program participants at the senior day care facility.
2) drafting care plans for initial and 6th month reassesment based on the primary care physician's H&P.
3) maintaining daily flow sheets and progress notes.
4) cooperating with a social woker and program aide as a team member.
Hours of work: 8am - 4pm or 7:30am - 3:30pm
