Job Title: Bilingual Spanish Case Manager

Location: Valencia, CA

Position Type: Full-Time/Part-Time, Weekends or Monday-Friday

Overview:

We are seeking dedicated Bilingual Spanish Case Managers to join our dynamic team based in Valencia, CA. This position will involve working closely with clients who require medical care coordination, housing assistance, and social support services. Ideal candidates will be compassionate, organized, and ready to make a significant impact in the lives of individuals and families in need.

Key Responsibilities:

Conduct comprehensive assessments of client needs to determine necessary support services related to health, housing, and social services.

Develop, implement, and review service plans in collaboration with clients and other professionals to ensure a holistic approach to care.

Provide direct support and coordinate services including scheduling appointments, facilitating transportation, and assisting with access to medical and social services.

Maintain detailed records of case management activities and client progress using our designated systems.

Serve as a liaison between clients, families, healthcare providers, social agencies, and other relevant parties.

Advocate for client access to community resources and services, ensuring clients receive optimal care and support.

Adhere to all confidentiality and privacy policies.

Travel as necessary to meet with clients, attend meetings, and coordinate services across various locations.

Qualifications:

Proficiency in Spanish and English is required.

Minimum of two years’ experience in case management, preferably within healthcare, social services, or a related field.

Demonstrated ability to work compassionately with diverse populations, including those with complex needs.

Strong organizational skills and the ability to manage multiple cases efficiently.

Excellent communication skills, both written and verbal.

Ability to travel as required for client visits and other job-related duties.

Familiarity with Microsoft Office Suite and case management software.
