In this role, you will provide essential support and advocacy to individuals and families in need, helping them navigate complex social, health, and housing systems. Key Responsibilities:
Assess clients’ needs and develop personalized care plans
Provide counseling, advocacy, and referrals to appropriate community resources
Assist clients in accessing housing, healthcare, financial assistance, and other services
Monitor client progress and adjust plans as necessary
Maintain detailed and accurate records of client interactions and services provided
Collaborate with social service agencies, healthcare providers, and other professionals to ensure comprehensive care
Ensure all work is performed in compliance with organizational policies and legal requirements.
Qualifications:
Bachelor’s degree in Social Work, Human Services, or a related field (required)
Previous experience in case management or social services (preferred)
Strong understanding of social service systems and community resources
Excellent communication, interpersonal, and organizational skills
Ability to work independently as well as part of a team
Compassion, empathy, and dedication to supporting vulnerable populations
Proficiency in documentation and case management software
Bilingual skills (a plus)
