Position Description

Title: Clinical Coordinator

Reports to: Executive Director

Hours: Full-time

Salary Range: $55,000 - $65,000 (salaried) DOE/Qualifications

General Duties and Responsibilities

Under the supervision of the Executive Director, the Clinical Coordinator provides day to day coordination and oversight of the services and performance of the Specialty Mental Health Services (SMHS).

The Clinical Coordinator will work the caseload assigned. This position is required to work collaboratively with other services and programs, program partners, and numerous community agencies. The Clinical Coordinator will assign work and standardize practice per corporate policies and scopes of work, working in close conjunction with the Executive Director.

Perform other duties as assigned.

Specific Duties and Responsibilities

Ensure that a safe, secure and welcoming environment is provided by maintaining safety protocols.

Provide supervision of care managers, including coaching and training in Evidence Based Practices and note-writing.

Provide performance management for all clinical SMHS staff against billable client service minutes targets.

Performance manage team productivity through assigning work and allocating clients, through use of motivational and time management tools, and monitoring of reports.

Oversee and ensure provision of an intentional clinical experience for each client, derived from the Care Plan, utilizing Evidence Based Practices.

Coordinate the provision of access to MCHC services through MCHC staff on-site and referral to other services, such as housing, education, welfare benefits & financial assistance.

Provide and ensure provision of appropriate billable services to client who meets medical necessity for SMHS per Title 9.

Undertake assessments and provide diagnoses.

Provide the full range of billable services, including but not limited to assessment, plan development, care management, rehab, linkage, collateral, therapy.

Complete all required record keeping, including Medi-Cal billing and electronic and paper-based case records. Keep records as required, including the preparations of B.P.S.A.s, Care Plans, D.P.U.S, and Progress Notes.

Provide documentation review including but not limited to review of Bio Psychosocial Assessments, Care Plans and Progress Notes.

Deliver the productivity rate for the position (currently 62% billable time).

Utilize outcome management tools and other electronic care management tools.

Provide information and referrals for clients eligible for other services in the greater Fort Bragg area and Mendocino County.

Participate in case conferencing meetings, HH/ HC staff meetings, and related training as scheduled.

Work to process and problem solve client grievances.

Maintain good working relationships with other service providers and other agencies.

Follow HH/ HC protocol for securing Release of Information from clients.

Maintain client confidentiality.

Maintain clients records and data, including inputting of data to the care management databases, including Medi-Cal related data.

Coordinate with other HH/ HC services.

Job Skills and Abilities

Licensed or waivered licensed clinician with the CA Board of Behavioral Sciences.

Experience of Medi-Cal billing including delivering billing targets

Understanding of substance use disorder and mental health diagnoses and treatments.

Self-starter, goal oriented.

Ability to exercise good judgment, follow oral and written directions.

Good clerical skills and attention to detail

Ability to communicate effectively both orally and in writing.

Demonstrated ability to establish cooperative working relationships with clients and staff.

Understanding of and ability to maintain client confidentiality.

Excellent listening skills.

Good computer skills including ability to use Office software, databases, and Internet.

Ability to maintain attitude of fairness and objectivity.

Knowledge of greater Fort Bragg and Mendocino County social and human service programs.

Valid California drivers license and clean DMV record
