Avalon Health Care Management, Inc. is seeking an enthusiastic Central Intake Coordinator to join our team!
The Centralized Intake Coordinator is responsible for providing referral sources one point of contact for multiple centers in a shared market. Position is responsible for coordination of referrals ensuring all pertinent information is obtained, documented and communicated to the assigned facilities.
Responsibilities
-Perform as primary resource for coordination of referrals in a responsive time frame with referral sources on behalf of assigned facilities
-Coordinate execution of steps to conversion with facilities to increase conversion ratio
-Establish and maintain sound working relationships with referral sources
-Utilize PCC IRM database to enter all referrals and outcomes
-Daily communication with assigned facilities to discuss bed management, open referrals and potential admissions
-Attend census calls and report necessary information related to call
-Support 24/7 admission coverage with team members at facilities
-Embrace Avalon sales philosophy and abide by all policies and procedures
-Knowledgeable of Resident Rights and assists residents exercise and/or protect their rights
-Follows and adheres the policies and procedures of the facility implementing HIPAA requirements for the privacy and security of protected health information
-Complies with laws and regulations applicable to position and acts in accordance with facilities compliance program
-Follow safety rules of the Center
-Other duties as assigned.
Qualifications
-High School Diploma or equivalent, Associates or in Health Care field preferred
-2+ years relevant work experience
-Exceptional communication skills with a variety of professional and staff level individuals
-Must meet all local health regulations, and pass post-employment background checks as required
-Must be capable of performing the Essential Job Functions of this job, with or without reasonable accommodation
-Demonstrates self-confidence.
-Ability to work flexible hours, as admission responsibilities may dictate.
