Position Overview:

We seek a dedicated and experienced Lead Care Manager for our (CS) and (ECM) departments. This virtual/hybrid role requires a dynamic individual capable of managing a diverse team, coordinating care services, and implementing effective strategies to meet the complex needs of our clients. The ideal candidate will have a strong background in social work, care management, or a related field, with a proven track record of leadership and a deep commitment to improving the lives of those in need.

Key Responsibilities:

Team Leadership and Management: Lead, mentor, and support a team of care managers and support staff, fostering a collaborative and positive work environment. Ensure high-quality service delivery and professional development opportunities for team members.

Client Care Coordination: Oversee the assessment, planning, and implementation of care plans for clients, ensuring that services are client-centered and address individual needs effectively.

Resource Development and Management: To enhance service delivery and identify, develop, and manage relationships with community resources and partners. Ensure clients access necessary services, including housing, medical care, food assistance, and emergency support.

Program Development and Evaluation: Collaborate with organizational leadership to develop and refine CS and ECM programs. Implement evaluation strategies to assess program effectiveness and identify areas for improvement.

Case Management Oversight: Ensure that case management practices meet organizational standards and best practices. Oversee the documentation and tracking of client progress, adjusting care plans as necessary.

Stakeholder Engagement: Serve as a key liaison between Mysti’s clients, families, and community partners. Promote the organization’s mission and services within the community and professional networks.

Qualifications:

Bachelor’s or Master’s degree in Social Work, Psychology, Public Health, or a related field.

Minimum of 3 years of experience in care management, social work, or a related field, with at least one year in a leadership role.

Strong understanding of the social services landscape in Monterey County and the specific needs of diverse client populations.

Excellent communication, interpersonal, and organizational skills.

Ability to work effectively in a virtual and hybrid environment, with proficiency in relevant technology and platforms.

Demonstrated ability to lead and motivate a team, manage multiple priorities, and develop collaborative partnerships.

Commitment to continuous learning and professional development in the fields of social work and care management.

Work Environment:

This position offers a flexible, virtual/hybrid work environment, requiring both remote work and, as needed, in-person engagement in Monterey County. Candidates must have a suitable home office setup and be willing to travel locally for client and community partner engagements.
