Qualifications

Preferred: Bachelor’s Degree.

Preferred Experience: Social Work, Nursing, Home Care, or Discharge Planning.

Required: Current State license & CPR Certificate.

Valid driver’s license, automobile insurance, and willingness to drive to client locations.

Strong verbal, written, and organizational skills.

Desired Qualifications:

Minimum of 2 years’ experience as a Care Manager or related role.

Extensive assessment and care planning skills.

Knowledge of geriatric care issues, family dynamics, interventions, education, training, supervision, quality

assurance, customer service, and marketing services.

Ability to interface effectively and professionally with clients' support networks including families, physicians,

attorneys, etc.

Promote services and programs to professional referral sources.

Proficiency with Microsoft Office Suite.

Job Description

The Care Manager is responsible for assessments, care coordination, advocacy, and related activities for an assigned

group of clients. This role involves developing individualized care plans, collaborating with primary physicians,

teaching client’s self-care, evaluating care responses, initiating referrals, and supervising care staff and family

members.

Client Assessments and Planning:

Conduct thorough assessments of clients' ADLs, IADLs, psychosocial and functional abilities, safety/home

environment, financial/legal status, and ongoing care needs.

Develop tailored care plans with desired outcomes.

Make recommendations and estimate care costs.

Identify and refer clients to specific resources such as home health, durable medical equipment, mental health,

psychosocial resources, transportation, medical housing, and educational materials.

Implementation and Monitoring of Care:

Arrange, direct, and monitor resources as they provide service to clients.

Advocate and coordinate among those involved in clients' care.

Documentation and Reporting:

Prepare written updates for clients and families.

Monitor clients' status, re-evaluate care plans, and provide monthly phone or email updates to clients and families.

Supervision and Training of Caregivers:

Coordinate care with the Branch Manager and Staffing Coordinator.

Address and maintain quality care proactively.

Assist with ongoing education for caregivers.

Document and update activities to ensure compliance with protocols.

Participate in mandatory agency staff meetings to maintain quality care standards.
