Job Title: Case Manager
Reports to: Executive Director
Education Requirement: Bachelors Degree
Experience Required: Minimum 1 year of human/social services program delivery
Position is: Exempt, Full Time or Contract
Job Summary:
Under the supervision of the Executive Director, the Case Manager is responsible for the screening, advocacy, and case management of the SCOC clients. The Case Manager is also responsible for data entry and data management related to their clients and other program participants, as determined by the Executive Director. As well as sourcing and placing clients in jobs and ensuring clients follow through on their goals and objectives. Case Manager must be able to cold-call over the phone and in person to find clients employment. Case Manager must understand drug/alcohol addiction and have a compassion for individuals who are experiencing homelessness and have mental illness.
Essential Job Duties:
Screening of Clients: (20% of time spent)
Collaborate with Partners to receive client referrals.
Visit Homeless Shelters to meet clients and work with Service Providers
Case Management Duties: (70% of time spent)
Coordinate and/or provide: life skills counseling, mentoring and support services, and referrals to community resources and find employment for clients.
Maintain an average size caseload of both active and pending clients.
Assess clients challenges, strengths, and areas where they may need assistance.
Develop an individualized service plan with monthly goal sheets for each client.
Meet with client weekly during initial period, or more if needed, to help them achieve their goals.
Coordinate resources and referrals with partners.
Keep and maintain appropriate records and complete all relevant agency/program forms, doing so in a confidential and professional manner.
Be responsible for timely entry of case notes, services and client information into Client Files and required data base systems.
Monitor all client expenses; seek approval for expenses if above amount approved.
Help clients coordinate transportation to job interviews
Additional Duties: (10% of time spent)
Report success stories, progress, and statistics to the ED
Fill in as needed in other areas as needed
Attend all staff, case management, and Board meetings as requested.
Might need to work evenings and weekends as required.
Additional Qualifications:
Maintain valid California Drivers License and State Mandated Auto Insurance
Must be able to utilize Word, Excel and Power Point
Compensation:
Range: Negotiable - But above average for non-profit industry
Phone or monthly phone stipend
Reasonable expenses
