Trusted Path Home Health is a Joint Commission–accredited agency dedicated to delivering compassionate, high-quality care in the comfort of patients’ homes. We are seeking an experienced Licensed Vocational Nurse (LVN) with a minimum of 1 year of home health experience to join our team.

Responsibilities
The LVN provides skilled nursing care under the supervision of a Registered Nurse (RN) and in accordance with the Plan of Care established by the physician and agency. Duties include:
Perform assigned patient visits, including follow-up skilled nursing visits.
Provide direct care such as wound care, catheter care, injections, medication administration, and vital sign monitoring.
Observe and promptly report patient conditions and changes to the supervising RN.
Assist in implementing and carrying out the physician-approved Plan of Care.
Educate patients and families on medication management, disease processes, and home safety.
Complete all clinical documentation accurately, timely, and in compliance with Medicare, state, and agency requirements.
Communicate effectively with patients, families, supervising RNs, physicians, and other interdisciplinary team members.
Support patients and caregivers to promote independence and positive health outcomes.

Qualifications
Current, active LVN license in the state of California.
Minimum of 1 year of home health care experience.
Familiarity with Medicare and home health documentation preferred.
Proficiency in wound care, medication administration, and patient education.
Strong communication, organization, and teamwork skills.
Reliable transportation and valid driver’s license.
Current CPR/BLS certification.
