Adult Day Health Care Program is a licensed community-based day health program that provides services to older persons and adults with chronic medical, cognitive, or mental health conditions and/or disabilities that are at risk of needing institutional care.
Social Worker duties include:
-Assessment of each participant
-Creating an individual plan of care for each participant
-Providing counseling, individual and group interventions according to the plan of care
-Providing referrals to available community resources
-Serving as a liaison for each participant
-Community liaison role, intake triage, and facility tours
-Maintaining social service records for each participant
Position qualification:
-Master’s degree in social work from an accredited university
-Bachelor’s degree in social work from an accredited university (qualifies for Assistant Social Worker position)
-CPR and First Aid certificate
