We are both a fast-growing Medicare Certified Home Health Agency based in
Sacramento and providing Skilled Nursing, PT, OT, ST, MSW and CHHA disciplines to
patients.
We are seeking State licensed Occupational Therapists to join our professional care
team and serve Sacramento and its environs.
Key Roles:
- Evaluate patient conditions and develop appropriate Occupational Therapy Plan of
Care
-Implement the treatment plan through direct occupational therapy sessions
-Supervise work of COTAs.
-Work cooperatively with other patient care team personnel
-Participate in quality improvement process
-Home Health experience an added advantage
-Satisfactory Criminal background check
