The Adult Day Health Care Program is a licensed community-based day health program that provides services to older persons and adults with chronic medical, cognitive, or mental health conditions and/or disabilities that are at risk of needing institutional care.
The Registered Nurse (RN) is part of multidisciplinary team (includes physical therapy, occupational therapy, speech therapy, medical social work, rehab activity).
The Registered Nurse’s (RN) job description includes conducting initial and periodic assessment of each participant, providing skilled nursing services according to individual plan of care, supervising services provided by licensed vocational nurses and program aides.
Full time (30 hours per week), part time, and on-call options are available. Bilingual candidates (Russian, Ukrainian) are preferred.
