CCP Services: Career Pathway Navigator
Position Summary:
Under the general supervision of the CCP Program Manager; the Career Pathway Navigator will follow the Independent Living philosophy to provide a variety of tailored, individualized supports to meet the changing needs and preferences of CCP clients by coordinating various opportunities for consumers by utilizing the Person Centered Career Plan. These Services include offering personalized guidance, resources, and information on various career pathways, including community integration, vocational training, postsecondary education, and career options, aiding individuals in shaping their unique trajectory. For individuals in secondary education, participating in IEP meetings upon request, collaborating with regional center service coordinators to facilitate the transition from youth to adult services, and partnering with school district representatives to promote paid work experiences during high school.
JOB SPECIFICATIONS
Required:
A Bachelors-level degree with 3 years’ experience in the field of developmental disabilities service systems or an Associates-level degree with 5 years’ experience. The CPN must have completed a course or training in person-centered thinking/planning and hold either an Association of Community Rehabilitation Educators (ACRE) certificate with an emphasis on Customized Employment
Ability to communicate effectively both orally and in writing.
Professional English writing ability, including usage, grammar, spelling, punctuation and vocabulary.
Ability to keep and file records.
Willingness to accept work direction from others.
Must be knowledgeable of the service delivery systems including the regional center and generic services for community integration and employment.
Proficient use of basic office equipment, including Microsoft Office Suite.
Immediate access to a vehicle, and proof of current driver’s license and car insurance.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
1. Crafting a Person-Centered Career Plan (PCCP) encompassing employment, community involvement, postsecondary education, and ancillary support, all geared towards achieving CIE. Collaborates with the Individual Program Plan (IPP) team, service coordinator, and individual to embed PCCP goals and measurable milestones into the IPP. Provides support for setting and tracking personal progress goals, and participates in IPP and IEP planning sessions upon request.
2. Offering personalized guidance, resources, and information on various career pathways, including community integration, vocational training, postsecondary education, and career options, aiding individuals in shaping their unique trajectory.
3. Facilitating interpreter services, translations, and creating plain-language materials as necessary for individuals and their representatives.
4. Supporting individuals and families in accessing and navigating employment-related services and supports from regional centers and generic sources to overcome barriers and fulfill specific needs like transportation.
5. Conducting monthly progress reviews and reassessments with individuals or their representatives to address any unmet needs. Adjusts the PCCP as required and expedites plan modifications.
6. For individuals in secondary education, participating in IEP meetings upon request, collaborating with regional center service coordinators to facilitate the transition from youth to adult services, and partnering with school district representatives to promote paid work experiences during high school.
7. Providing quarterly reports on achieved milestones, rendered services, employment outcomes, and an annual progress report. Communicates updates to the IPP or referrals necessary to achieve employment goals with the service coordinator.
8. Assisting individuals in personalized benefits planning and providing tools for financial empowerment.
9. Collaborating with the service coordinator to support the transition to ongoing individual supported employment services or requesting an extension of CCP based on individual needs.
PERFORMANCE STANDARDS
Provides effective instruction to participants in order to achieve their PCCP goals and objectives.
Timely, accurate and complete documentation of progress reports
Provides instruction to participants at Home/Community sites in the manner most effective to the consumers, including: demonstration, preferred language, verbal and non-verbal cues, and visual aids.
Meets all safety standards established by SCRS, including safety drills and procedures with wheelchairs.
Meets employer dress code standards.
Engages actively with consumers at Home/Community sites to ensure that the participants are sufficiently served so as to meet CCP quality requirements.
Attends all mandatory meetings, including: staff meetings, all-staff meetings, scheduled trainings, and meetings with managers.
Maintains professional boundaries with consumers and co-workers.
Collects and records data daily on progress toward achievement of participant CCP goals and objectives.
Records all other relevant information in progress notes.
Maintains participant confidentiality with parents/caregivers and Employees.
Maintains professional relationship with parents/caregivers.
Follows CRS policies and procedures on employee attendance and tardiness.
Maintains participant files and records, according to agency procedures.
Provides appropriate level of administrative support to Program Manager.

$25.00/Hr Without ACRE / Certification
$26.00/Hr With ACRE Certification
